FC Drive | Registration and Club Documents
To all Players and Parents,
Congratulations. Welcome to FC DRIVE!
Please first read the FC DRIVE 2008 Family and Finance Agreement and Illinois Youth Soccer Medical Release (found below the registration details).
ONLINE REGISTRATION
Registration is on-line in the secure registration site. Registration must be completed by June 13. If you cannot complete registration by that date or encounter any difficulty, email fred@fcdrive.net. If registration is not completed, roster spots may be released to alternates.
- Select SAFE LOGIN from the menu on the left.
- Login with the user name and password created during your tryout registration.
- You will be asked to review your family information, player information, check certain boxes, and upload a color photo of your player’s face (if it is not already there). Photos must clearly show the player’s face only (no sunglasses and no hat). Other photos will not be accepted. The leagues are very strict on photo acceptability.
- Payment may be made by credit card during the registration process.
If payment is made by credit card
- For all players: please mail 2 signed, completed copies of the 2008 Illinois Youth Soccer Emergency Medical Release & Liability Waiver form (download below) to FC DRIVE, Registration Materials, 106 West Germania Place, Box 257, Chicago, IL 60610.
- For new players only: also mail proof of age (birth certificate, passport, state ID).
- For girl players only: also mail a signed, completed 2008 Illinois Women’s Soccer League Player Registration form.
- All documents must be postmarked by June 15.
IN-PERSON REGISTRATION
Registration may be completed in person at Margate Field House, 4921 N. Marine Drive (between Lawrence and Foster, 1 block west of Lake Shore Drive) on Wednesday, June 11 or Thursday, June 12, 6:30pm-8:30pm. Registration must be completed by June 13. If you cannot complete registration by that date or encounter any difficulty, email fred@fcdrive.net. If registration is not completed, roster spots may be released to alternates.
Please bring
- A signed, completed 2008 Family and Finance Agreement,
- 2 signed, completed copies of the 2008 Illinois Youth Soccer Emergency Medical Release & Liability Waiver form (download below),
- A passport sized, color photo of your player’s face. Photos must clearly show only the player’s face (no sunglasses, no hat). Other photos will not be accepted. The leagues are very strict on photo acceptability.
- Check/cash/money order/credit card.
- For parents of girl players only: bring a signed, completed 2008 Illinois Women’s Soccer League Player Registration form.
- For new players only: bring a copy of proof of age (birth certificate, passport, state ID).
