FC Drive | Registration and Club Documents
To all Players and Parents,
Congratulations. Welcome to FC DRIVE!
Before registering, please read the FC DRIVE 2009-10 Family and Finance Agreement (download below).
PLAYER REGISTRATION (all forms may be downloaded below)
Parents of Boy Players: please
- complete and sign 2 copies of the US Club Soccer Youth Club Registration Confirmation,
- complete and sign 1 copy of the FC DRIVE 2009-10 Family and Finance Agreement,
- provide 1 passport sized color photo of player's face (no hat, no sunglasses),
- provide a copy of government issued proof of age for new players only, and
- provide payment by check or money order payable to FC DRIVE.
Parents of Girl Players: please
- complete and sign 2 copies of the Illinois Youth Soccer Emergency Medical Release,
- complete and sign 1 copy of the Illinois Women’s Soccer League Player Registration Form,
- complete and sign 1 copy of the FC DRIVE 2009-10 Family and Finance Agreement,
- provide 1 passport sized color photo of player's face (no hat, no sunglasses),
- provide a copy of government issued proof of age for new players only, and
- provide payment by check or money order payable to FC DRIVE.
EMAIL OPTION
Scan forms, attach forms and digital photo to an email, send to dana@fcdrive.net;
mail or drop off payment (see below).

Drop off at the above address (one block south of North Ave and Clark St; across from Latin High School) Monday-Friday 8am-7pm, Saturday 9am-4:30pm
