To all Players and Parents,

Congratulations. Welcome to FC DRIVE!

Please read Travel Soccer Section--Registering on a Team, then

Complete ONLINE and OFFLINE Registration Components:

 

ONLINE REGISTRATION COMPONENT

If you are a returning player and created an account last year, please update your account here.

If you created an account by registering for the 2011-12 tryouts, please complete that account here. 

When completed, your account can accept online tuition payment with a Visa or MasterCard.

If you did not register online for 2011-12 tryouts, click here to create an account.

After your account is created, contact dana@fcdrive.net before returning to your account.

 

Online Uniform and Equipment Ordering

Training shirts are required for each player.  Order here

Order Adidas uniform kits for new players, optional and/or replacement equipment here

 

OFFLINE REGISTRATION COMPONENT (all forms may be downloaded below)

Parents of Boy Players: please

  1. complete and sign 2 copies of the US Club Soccer Youth Club Registration Confirmation,
  2. complete and sign 2 copies of the Illinois Youth Soccer Emergency Medical Release,
  3. complete and sign 1 copy of the FC DRIVE 2011-12 Family and Finance Agreement,
  4. provide 1 passport sized color photo of player's face (no hat, no sunglasses),
  5. provide payment by check payable to FC DRIVE, cash, or online using Visa/MasterCard, and
  6. for new NISL players only:  provide 1 copy of government issued proof of age

Parents of Girl Players: please

  1. complete and sign 1 copy of the Illinois Women’s Soccer League Player Registration Form,
  2. complete and sign 2 copies of the Illinois Youth Soccer Emergency Medical Release,
  3. complete and sign 1 copy of the FC DRIVE 2011-12 Family and Finance Agreement,
  4. for U09G and high school girl parents only: complete and sign 2 copies of the US Club Soccer Youth Club Registration Confirmation,
  5. provide 1 passport sized color photo of player's face (no hat, no sunglasses)
  6. provide payment by check payable to FC DRIVE, cash, or online using Visa/MasterCard, and
  7. for new NISL (U09G and high school girls only) or new IWSL players only:  provide 1 copy of government issued proof of age.
